Your individual responses will not be shared with anyone. RAE will use the results of this survey to make informed and effective decisions about Paragon^®.

• If you fully complete the survey, you can choose to enter your name for a prize draw.
• Prizes include:

• If you wish to be entered into the draw, we will need your name and contact information. You may also choose to complete the survey without being entered into the contest, and you do not have to provide your name and contact information.

All responses are anonymous and are treated confidentially. The privacy of your responses has been protected in a number of ways:

• An external consultant, Framework Analytics Inc., has been contracted by RAE to conduct this survey.
• Framework Analytics Inc. is collecting and analyzing the results.
• Framework Analytics Inc. is fully compliant with privacy legislation.
• This survey is conducted in compliance with the Marketing Research and Intelligence Association's Code of Ethics.
