We recognize that your time is valuable, and we appreciate your participation. Consequently, we have kept the survey as short as possible while still achieving the information objectives set out for the project.

If you wish to be entered into the draw, we will need your name and contact information. You may also choose to complete the survey without being entered into the contest, and you do not have to provide your name and contact information. 

Respondents who fully complete the survey will be offered the opportunity to be entered into an incentive draw for one of the following prizes:

• One $500 Everything Gift Card;
• One $200 Everything Gift Card;
• One pair of tickets to RAEcon on September 17, 2024.

Please be candid and forthright and respond to all questions, even if your response is “Not Able to Assess” (Don’t know or Not Applicable).

Your individual responses will not be shared with anyone. RAE will use the results of this survey to make informed and effective decisions about Paragon®.

All responses are anonymous and are treated confidentially. The privacy of your responses has been protected in several ways:

• An external consultant, Framework Analytics Inc., has been contracted by RAE to conduct this survey.
• Framework Analytics Inc. is collecting and analyzing the results.
• Framework Analytics Inc. is fully compliant with privacy legislation.
