We recognize that your time is valuable, and we appreciate your participation. Consequently, we have kept the survey as short as possible while still achieving the information objectives set out for the project.

We require your name and contact information to enter the draw. However, you may also choose to complete the survey without entering the contest, and you are not required to provide your name and contact information.

Respondents who fully complete the survey will be offered the opportunity to be entered into an incentive draw for one of the following prizes:

• GRAND PRIZE: An Apple iPad
• SECOND PRIZE: One (1) of fifty (50) $10 coffee cards

Please be candid and forthright, responding to all questions, even if your response is “Not Able to Assess” (i.e., “Don’t know” or “Not Applicable”).

Individual responses are confidential and will not be attributed. GVR will use the results of this survey to make informed and effective decisions about Paragon®.

All responses are anonymous and are treated confidentially. The privacy of your responses has been protected in several ways:

• An external consultant, Framework Analytics Inc., has been contracted by GVR to conduct this survey.
• Framework Analytics Inc. is collecting and analyzing the results.
• Framework Analytics Inc. is fully compliant with privacy legislation.
• This survey is conducted in compliance with the Canadian Research Insights Council’s (CRIC) Code of Ethics and
• Responses will be grouped. No one’s responses will be attributed.
